Our Doctor's are board certified in Family Medicine, Internal Medicine, Podiatry-Wound Care and Gerontology. They have extensive care with the adult and geriatric population. They have over ten years of experience in Hospital services as well as Rehabilitation services. 

With our practice being a Multi-Specialty group, our Board-Certified Physicians will provide a comprehensive home care evaluation including diagnosis and treatment for ailments and can provide same day care with our Cardiologist-Electrophysiologist, Podiatrist or Neurologist if needed. We also offer standard check ups including but not limited to the following

• Physical Exams
• Medication Management
• Chronic care management
• Blood draw, Vacci
• Transitional care management after hospital or rehab stay

Our group offers hospitalist services and we can work closely with a team to admit care in a seamless way and ensure continuity. We stay in close contact with the admitting physician or can provide care with our Hospitalist doctors. We will assure care is resumed once the patient is discharged from the hospital.

We can prescribe, monitor or adjust your medications as necessary to assure safe and continuous management of you or your loved ones medications

Home visits are traditionally covered by Medicare, Horizon BCBS, Aetna Medicare, Wellcare and more. Patients with supplemental insurance will typically cover the 20% not covered by Medicare and the annual deductible. We also accept Private Pay with payment plans as well. 

We offer patients Telehealth visits in addition to our home-visits. It can allow us to care for you or a loved one in a HIPAA compliant video-chat. We can also screen via this tool in order to see if a home-visit is needed.

While we do work alongside your specialists and ours to manage your care, we do recommend utilizing our primary care services as your primary doctor so we would become your primary care physician. Continuity of care is the safest and best care!
